Bivalent COVID-19 Vaccine Booster Clinic at the NIU Convocation Center

https://app.acuityscheduling.com/schedule.php?owner=21571681&appointmentType=37054710The DeKalb County Health Department will host a bivalent COVID-19 vaccine booster clinic at the NIU Convocation Center from noon to 3 p.m. Friday, Sept. 30.
Before you schedule your bivalent COVID-19 vaccine booster appointment, ensure you have waited the appropriate amount of time (refer to your vaccine record card). Bivalent vaccines can be used as a single booster dose at least two months following a primary series or booster vaccination.
Bivalent COVID-19 boosters have been authorized for people who have completed the primary series and people who are:
- Ages 12-plus (Pfizer-BioNTech).
- Ages 18-plus (Moderna).
Please note that boosters from the original formulation (monovalent mRNA) are no longer authorized for use as boosters in people ages 12 and older. Therefore, walk-in appointments for COVID-19 boosters for those 12 and older are not available at this time.
If appointments are no longer available, please check the vaccines.gov website to view additional provider locations where you can receive the bivalent COVID-19 vaccine booster.
On the day of your appointment:

Wear a short-sleeve shirt to ensure the vaccine is easily administered.

Review the EUA's for the vaccine you are receiving (Pfizer- BioN Tech or Moderna).

Wear a face covering.

Bring a photo ID.

Complete the consent form and bring it with you at your scheduled appointment time. Bringing your completed form will help expedite the vaccination process.

Bring your COVID-19 vaccination card.
