Attention 2Ls (December 2021 and May 2022 graduates) - Required meeting to complete a Graduation Requirement Checklist:  

• Email Julie Mahoney-Krzyzek (jmahoney@niu.edu) as soon as possible to set up an appointment.   
• Appointments will be available between Monday, February 8, 2021 and Thursday, March 11, 2021.
• Office hours are 8:00am – noon and 1:00pm – 4:30pm Monday-Friday. (excluding Thursday, February 11).
• Meetings will be virtual (either via Teams or phone).
• Please include several fifteen minute timeslots that you would have free to be able to meet. Julie will respond back to you in email to confirm with you which time works for her schedule.
• IMPORTANT: If this form is not completed by the deadline, a hold will be put on your account and you will not be able to enroll in Summer 2021 or Fall 2021 courses.
